At Evolve Chiropractic, Dr. Anselmo is an out-of-network provider. Payment is required at the time of service, but a patient can be provided with a superbill to submit to their insurance for reimbursement. By not being an in-network provider, Dr. Anselmo is not limited by insurance deciding how many times a patient can be seen and the quality of care that is able to be given. HSA is also accepted.

INITIAL VISIT
40-minute appointment consisting of understanding the patient's health history, an exam, questions/discussion about what we are doing and our goals, and a chiropractic treatment. If indicated after the history and exam portion of the visit, the treatment includes soft tissue work and a chiropractic adjustment.

FOLLOW-UP VISITS
20-minute chiropractic adjustment and soft tissue work as needed.
